Our Cancellation Policy:
- 24-Hour Notice Required: Please provide at least 24 hours’ notice for appointment cancellations
- Late Cancellation Fee: $175.00 fee may apply for cancellations with less than 24 hours’ notice
- No-Show Fee: $175.00 fee applies for missed appointments without prior notice
- Emergency Exceptions: We understand that true emergencies occur and will work with you on a case-by-case basis
7. Intellectual Property
All content on our website, including text, images, logos, and design elements, is owned by Oak Tree Behavioral Services or licensed for our use. You may view and use this content for personal, non-commercial purposes only.

11. Insurance & Payment Terms
Insurance
- We are in-network providers with majority of insurance plans
- Insurance benefits are verified through our Simplelntake system
- You are responsible for understanding your benefits and any applicable deductibles or co-pays
- This practice does directly bill to any insurance or medical plan;
Payment
- Payment is due at time of service unless other arrangements have been made
- We accept Visa, Mastercard, American Express, Discover, and HSA.
- Outstanding balances over 365 days may be subject to being sent to collections.
12. Termination
We may suspend or terminate your access to our website or services if you violate these terms or our clinical policies. You may discontinue services at any time, though we encourage discussing termination in session when clinically appropriate.
